This tag is made in Paint Shop Pro 9, but will probably work in any version.
It is my own idea, and any resemblance to other tuts is purely coincidental.

You will need a tube or an image of your choice. I'm using the art of Hallan Moulin. He is FTU but please read and follow his TOU!
You will also need Mura Meister Copies and EyeCandy 4000 and a font of your choice. (in my links section you can find links to plugin sites)

Open a new image 600x400pixels, transparent bg.
Pick a light foreground colour and a darker background colour from your tube.
I used #ba7f98 and #332d39.

Paste your tube/image as a new layer.
Go to Mura's Meister - Copies with the following settings

Effects - Distortion effects - Pixelate with these settings

With your pen tool set to 5 pixels and your light colour, draw a line covering each of the edges between the pixel squares, both horisontally and vertically.

Convert to raster layer.
Crop your image to get rid of anything outside the image.

Selection - select all - float - defloat.
Activate your pixelated layer and hit delete. Deselect.
Move the pixelated layer above the lines layer.
Add drop shadow with the following settings. Apply the drop shadow twice, but the second time, change the settings to H-2 and V-2.

Minimize this image for a while, and open your tube/image.
Resize your tube/image to 300 pixels high.
Image - add borders. Give it a 5 pixel border in the light colour (the same colour you used for the lines).

If you are using a tube without a background, you will automatically get a white bg when adding borders this way.
If you wish to have a different bg colour, simply add a bg layer in your prefered colour below your tube
and merge visible before adding the border.
Copy.

Open your pixelated image again and paste the framed tube as a new layer.
Place it where you think it looks nice.
Add a drop shadow. I used 2,2,35,5,black and applied it twice.

Switch to your rectangular shape tool.
Draw out a rectangle around your framed image, using these settings.
The right edge of the rectangle should be outside the workspace.

Convert to raster layer.
Crop your tag like before.
Add a drop shadow. I used the same as before.

Add a new layer. Fill with your light colour.
Selections - select all - Modify - contract 5 pixels. Delete. Deselect.
Add the same drop shadow as you added to the pixelated layer.

Now it's time to add your name.
Pick a nice font and write the name you want to put on the tag.

I used Pharmacy which you can find here.
Objects - Align - center in canvas.
Convert to raster layer.
Effects - EyeCandy 4000 - Gradient Glow with these settings. Glow colour the same as your foreground colour.

Image - Rotate - free rotate.

Place the name where it looks nice on your tag.
Add a drop shadow like before.

Add your credits and watermark and save as .jpg and you're done!
